This policy describes information collected by TradeHealth, LLC through this marketing website. It does not describe patient care platforms or replace a clinical provider’s privacy notice.

Information you provide

Employer and partner inquiry forms collect your name, organization, email address, phone number, state, business or partner type, relevant workforce or client counts and any optional message you provide. Employer inquiries may also include your current benefits, referral source, referral code and membership interest. We record your agreement to be contacted.

How information is used

We use inquiry information to respond, discuss service availability and membership or partnership options, and follow up on your request. Submitting an inquiry does not enroll anyone in care or create a medical appointment.

Hosting and form processing

This website uses Netlify for hosting and Formspree for inquiry form processing. When you submit a form, your inquiry is sent to Formspree and made available to TradeHealth through its Formspree account and configured email notifications. Authorized TradeHealth team members may access it to handle your request. Hosting and anti-spam services may process technical information such as IP addresses, request data and browser information for delivery, security and abuse prevention. Cookies and browser storage

This website does not add advertising or analytics trackers and does not store form entries in browser storage. The pricing calculator runs in your browser; its inputs are not submitted as an inquiry. Hosting and security services may process technical data needed to operate the site.

Keep health information out of these forms

Do not send symptoms, diagnoses, prescriptions, medical records or other patient information through this website. These forms are for business inquiries only. For an emergency, call 911.

Retention, choices and requests

We keep inquiry information as needed to respond, maintain the business relationship and meet applicable legal obligations. You may ask about your information, request correction or deletion, or ask us to stop follow-up by replying to a TradeHealth response or using the inquiry form with your name, email and business name. Please do not include sensitive information. Applicable law may affect which requests we can fulfill.

We may update this policy as the website or services change. Links to other websites are subject to those websites’ policies. No internet transmission or storage method can be guaranteed completely secure.
